The Freedom of Information Act (FOIA) governs the inspection of public records. The Act requires that all public bodies make their public records available, with the exception of specific exemptions as defined by state statute.
To assist us with fulfilling your request for public records from the Village, please download the following FOIA Request Form (PDF), complete it in its entirety, and submit it to the Freedom of Information Act (FOIA) Officer. You are also able to submit a request in writing without using the "Request Form." Village staff will reply within the time period defined by statue.
Forms can either be faxed, emailed or mailed to the appropriate department.
